NYC Care
NYC Care is a health care access program that guarantees New Yorkers who do not qualify for/cannot afford health insurance low-cost and free services listed below. All services offered by NYC Care are provided through NYC Health + Hospitals.

Enrollment Process
Certain ways to enroll include:
Meet with an NYC Health + Hospitals Financial Counselor
Meet with or call a MetroPlus representative
Go to one of their approved community-based partners
Documents required for Enrollment
You will need proof of:
Identification
New York Residency
Household Income
Household Size
Patients who have already been screened for insurance through the NYS of Health Marketplace can bring a printout of the results of the determination and will not need additional documentation.

Upon approval, you will receive a membership card to access health services, choose your doctor, and get affordable medications. NYC Care can help you choose a primary healthcare provider as well. New NYC Health + Hospitals patients will have their first appointment within two weeks of entering the program.
Languages offered for services
NYC Care provides services in many languages, including written translations and in-person or telephonic interpretation. Currently, interpretation services are available in over 250 languages and dialects at NYC Health + Hospitals.
Eligibility
Live in one of the five boroughs of New York City.
Not qualify for any health insurance plan available in New York State.
Not be able to afford health insurance based on government guidelines.
Cost of Services
You will pay based on what you can afford. Discounted fees are determined by your family size and income and start at $0. There are no membership fees, monthly fees, or premiums. You will be asked to pay according to your household size and income for clinic visits, emergency room visits and hospital stays. You will receive affordable health care regardless of your immigration status or ability to pay.

Services are offered at any NYC Health + Hospitals patient care location in the five boroughs including 11 hospitals and dozens of community-based health centers.
